Balancing Health at Home Program

Our BH2 program focuses on activities that emphasize collaboration of care with individualized goals and interventions to meet each patient’s care needs. This includes prevention of return visits to the Emergency Department (ED) or hospital. Scales Home Health team members will work with your physicians, other care providers, and your caregivers to achieve this goal.
